On upgrades, customers are seeing foreign key issues with the HQU plugins, which causes the HQU plugins no to load correctly.

Description

We have seen two cases on customer sites where after the upgrade their are foreign key constraint violations on the HQU plugin tables.

This needs to be investigated and resolved, as this can be a headache to customers.

There is a workaround (disable the foreign keys on the HQU tables, remove the contents of the tables, and enable the foreign keys, then restart the server), but we should not have to have customer use the workaround.

We are now explicitly removing the attachment points, instead of relying on cascading.
